On Sunday, August 14, 2011, at about 9:00 p.m., members of the Albany Police Department responded to a home in the 1600 block of SW 27th Avenue in Albany to conduct a welfare check.
The Albany Police Department had received a report that the resident, 41-year-old Corey John Simons, told an acquaintance he wanted to harm himself.
After officers on the scene heard a gunshot from inside the residence, the Linn County Regional SWAT team was called in to handle the tactical scene. Members of the Oregon State Police also responded and provided investigative support.
Corey Simons, a Sergeant with the Oregon State Police assigned to the Salem Area Command Office, ultimately surrendered without incident on Monday, August 15, 2011, at about 12:30 a.m. The investigation revealed that Corey Simons, who was home alone, had fired a handgun and that the bullet exited the front of the residence.
